1. Navigate to the Information section and select the Cause Determination tab.

This tab will only populate when certain criteria is entered: Fire or HazMat Released.
- From the Response section, when Primary Incident Type Found is within the Fire category.
- From the Size Up section, when HazMat Released is indicated with Special HazMat actions required or spill >= 55 gallons.
2. From the Cause Determination you are able to document the Cause of Ignition by using the dropdown or list view.
3. An Investigations section may populate pending on the Cause of Ignition.
- Failure of equipment or heat source and Act of nature are the only two that will not populate the Investigations section.
4. Document any pertinent information using the dropdown view.

If fields are multiselect and using the dropdown select choices then click outside the area to apply those choices.
5. Use the list view by selecting on the icon and the list will populate to the right of the screen.

If the list view is multiselect click on all that are needed and select Apply.
6. When enabled you are then able to document the Factors that are affecting Fire Suppression efforts.
7. Document if any, the number of rooms and/or stories damaged.
8. When a numeric value is entered you are able to then select an amount of damage.
9. When documenting the first item ignited responsible for flame spread as No will populate two other fields to document: Item Contributing Most to Flame Spread and Type of Material First Ignited.
10. When documenting a HazMat Cause Determination use the dropdown or list view to select the Cause of HazMat Release.
11. Document if a fire or explosion is involved with the incident.
12. When Are there factors contributing to HazMat Release? is enabled a new field will allow you to document any factors contributing to the release.
13. When Are there factors that affect mitigation efforts? is enabled a new field to document any factors affecting the mitigation.
