1.  Create a new form or select edit to edit an existing form.






2. Mouse over in the left margin at the current block and select the + sign to add a Persons Served Block under the current Block.





3.  Select Persons Served from the dropdown menu.






4. Select Tune to:
  1. Convert to:
    1. Text
    2. List
    3. Quote
    4. Checklist
    5. Heading
  2. Move up and down
  3. Delete (select the X 2 times).

  • Enable Service To feature before adding Person Served Blocks to avoid rendering issues
  • Position Person Served Blocks logically within your form flow
