1. Create a new form or select edit to edit an existing form.
2. Mouse over in the left margin at the current block and select the + sign to add a Permit Signature Block under the current Block.
3. Select Permit Signature from the dropdown menu.
4. Select "Tune" (#1) and then select "+" (#2) to add a new Signature Block.
