Purpose Statement
This article explains how administrators add, move and remove the Addendum table in First Due's Form Builder. The Addendum table gives a form a dedicated place to record extra information after the main form content, such as clarifications, corrections or notes. This keeps supplemental details organized in one place on the record, so they don't get scattered across free-text fields.
Background Information
- What it is: The Addendum table is a prebuilt table element in the Form Builder that you can add to any section of a form.
- Why it matters: Forms often need extra information added after they're first filled out. With a dedicated addendum area, that information is easy to find and stays clearly separate from the original entries.
- Common use cases:
- Adding clarifications or corrections to an inspection or report
- Recording follow-up details after the initial visit
- Documenting extra notes that don't fit the standard fields
- Prerequisites:
- Access to the Form Builder in the module you're working in (see Required Permissions)
- An existing form to edit, or access to create a new one
- A plan for where the table should go in the form layout
Required Permissions
- Form Builder: Read, Update and/or Create
- Read: view forms in the Form Builder
- Update: edit existing forms, including adding, moving and deleting the table
- Create: create new forms
To check or grant access:
- Click the First Due logo to open the module stack.
- Select Admin, then select Roles & Permissions.
- Open the user's role and set the access levels for the Form Builder permission.
Note: Changes to a form affect every user who fills it out.

Best Practices
Do:
- Put the Addendum table near the end of the form, or next to the content it supplements, so the form flows logically.
- After you add the table, check that it shows the fields you expect.
- Save your form regularly as you build it.
- Test the form on both web and mobile before rolling it out.
Don't:
- Don't add more than one Addendum table to the same form unless your workflow needs it.
- Don't remove the table from a form that's in use without considering how that affects existing records.
Common mistakes to avoid:
- Putting the table in a section that users rarely open
- Clicking Delete only once and assuming the table was removed. Deleting it takes a second click to confirm.
- Leaving the Form Builder without saving your changes
