Purpose Statement
The purpose of completing an evaluation is to document a candidate’s performance against assigned checklist items, rubric criteria, and overall pass/fail requirements.
Evaluations help departments maintain consistent training documentation, capture evaluator feedback, record required signatures, and support accurate training records within First Due.
Background Information
Evaluations are completed from the Evaluations module using the Evaluation List. Evaluators work through checklist items, document comments, add attachments, track attempts, and provide an overall pass or fail decision.
Depending on the evaluation setup, an evaluation may include a timer, rubric scoring, critical fail items, multiple attempts, required feedback, and required signatures before completion.
Evaluations may be saved while in progress and completed once all required items, feedback, and signatures have been entered.
Required Permissions
Users completing evaluations should have permission to access the Evaluations module and complete assigned evaluations from the Evaluation List.
Permission names may vary based on agency configuration. Confirm the appropriate Evaluation-related permissions with your department administrator.

Step-by-Step Guide
1. Navigate to the Evaluation List
- From the main menu, navigate to Evaluations.
- Select Evaluation List.
2. Open the Evaluation
- Locate the evaluation you need to complete.
- In the Actions column, select the Evaluate icon.
- The evaluation checklist opens.
3. Review the Evaluation Timer and Progress
- At the top of the evaluation page, review the timer if one is included.
- The timer begins based on the settings established during evaluation setup.
- Use the available timer options as needed:
- Pause the timer
- View the time log
- Reset time, if appropriate
- Review the progress tracker to monitor how many checklist items have been completed.
4. Complete Checklist Items
- Begin completing each checklist item as indicated.
- Enter evaluator comments when additional documentation is needed.
- Add attachments when supporting files or documentation are required.
- If the checklist item allows multiple attempts, select the additional attempt option to record another attempt.
5. Complete Rubric Scoring, If Applicable
- If the evaluation uses a rubric scoring method, score the checklist item using the available rubric scale.
- Rubric-based checklist items use a 1–5 scoring method instead of standard question types.
- Continue scoring each rubric item until all required items are complete.
6. Manage a Critical Fail, If Applicable
- If a failed item is marked as a Critical Fail, a modal opens.
- Select the appropriate option:
- End the Evaluation Now
- Continue and Document
- Retry, if the candidate should receive another chance to pass the item
7. End the Evaluation Due to a Critical Fail, If Needed
- Select End the Evaluation Now.
- Review the confirmation modal indicating the evaluation is ending due to a critical fail.
- Enter the required signature.
- Confirm that once the evaluation is ended, it will be locked and cannot be modified.
8. Choose the Next Action After a Critical Fail
Once the critical fail is confirmed, select the appropriate next action:
- Mark the evaluation as a failed evaluation.
- Schedule a reattempt.
- Immediately start a reattempt.
9. Provide Candidate Feedback
- Once all checklist items are complete, enter feedback about the candidate’s performance.
- Include specific comments supporting the pass or fail decision. (Optional Field)
- Select Pass or Fail based on the overall evaluation result.
- Select Save Evaluation.
10. Complete Required Signatures
- After the evaluation is saved, the signature section becomes available.
- Complete all required signatures.
- Select Complete.
11. View the Evaluation Summary
- Return to the Evaluation List.
- Select the evaluation summary icon to view the completed evaluation summary.

- Complete checklist items in order to help ensure the evaluation is documented consistently.
- Use comments to explain performance details, especially when marking an item as failed.
- Add attachments when additional documentation supports the evaluation result.
- Review the timer and time log before completing the evaluation if timed performance is required.

Troubleshooting & FAQs
Why can’t I complete the evaluation?
The evaluation may still have incomplete checklist items, missing feedback, or missing required signatures. Review the checklist, feedback section, and signature section before selecting Complete.
What happens if I save an unfinished evaluation?
The evaluation can be saved and returned to later. The system also performs an auto-save process as the evaluation progresses.
What does the timer do?
If included in the evaluation setup, the timer tracks the time spent completing the evaluation. Evaluators may be able to pause the timer, view the time log, or reset the time.
What should I do if a checklist item allows multiple attempts?
Use the additional attempt option to document each attempt when the evaluation setup allows multiple attempts.
What happens when a Critical Fail is selected?
A modal opens and the evaluator can choose to end the evaluation, continue and document the result, or retry the item if another attempt is appropriate.
Can an evaluation be edited after it is ended due to a Critical Fail?
No. Once the evaluator confirms the evaluation is ending due to a critical fail, the evaluation is locked and cannot be modified.
Where can I view the completed evaluation summary?
The evaluation summary can be accessed from the Evaluation List by selecting the evaluation summary icon.
