What is Form Language?
Form Language is the information displayed to a person signing (patient, crew member, nurse, etc.) when the signature box is presented to the signer. Use Form Language to create forms for each situation in which a signature may be obtained.
*** NOTE ***- A Form Language item needs to be completed for each Reason Signing for those reasons to show up in the signature block.
Directions:
1. Navigate to EMS Setup -> Signature Setup
2. Use the Signature Blocks tab to add or edit each block.
3. After defining the Block Name, choose which type of person(s) can sign within the selected block.
Configuring the Types of Person(s) signing in Setup will result in more accurate documentation by limiting the provider's choices in each block. Inside an ePCR, this configuration looks like this:
4. Choose which signature reasons that apply to the selected block.
Inside the ePCR:
5. Enable or disable Optional Fields within the selected block.
If the crew member field is enabled, providers will be able to choose a name from a dropdown roster list.
6. If default values are needed in the signature block, select from drop-down list.
7. Choose to enable or disable signature edits for the selected block after the report is completed.
8. Click on Save.
9. Use the Default toggle for blocks that should always be visible inside an ePCR.
Defaulted Signature Blocks are always visible. Any signature block that is not defaulted can be chosen by the provider at any time by clicking on "Add Signature Block". Use Default for the most commonly obtained signatures.
10. Use the Form Language tab to build out department specific verbiage that will be displayed when a provider obtains a signature during documentation.
11. Associate a form language with the appropriate signature reason and language. Next, copy/paste or type the language to be displayed in the text box.
