Purpose

  1. The purpose of this article is to demonstrate the documentation of an EMS patient within a fire report.

Directions


1. From the People Involved tab within the People Involved section began by adding a person using the +Add button or the expand arrow to add from occupancy.





2. From the modal when selecting the Civilian, EMS Patient the EMS tab will become active.
  1. Before selecting the EMS tab remember to gather pertinent information within the General Info section.

3. From the EMS tab you are now able to document all pertinent information needed.

4. When the Injuries toggle is enabled you can begin document the injuries by selecting the +Add button.





5. From the Injury modal now use the dropdown or list view to document the Body Site and Injury Type.

6. Using the dropdown or list view to document Human Factors, Other Factors, and Cause of Illness/Injury.
  1. If the Injured toggle is not enabled the Other Factors field will not populate.





7. To began documenting Treatments/Procedures select the +Add button.





8. From the modal enter any pertinent information using the fields.

9. Once treatments/procedures have been documented you are able to edit or remove.
  1. From the Actions column you can edit by selecting the pencil icon, to delete select the trashcan icon.
  2. To bulk delete treatments/procedures use the check box then select the trashcan icon.





10. To began documenting Medications select the +Add button.





11. From the modal enter any pertinent information using the fields.

12. Once medications have been documented you are able to edit or remove.
  1. From the Actions column you can edit by selecting the pencil icon, to delete select the trashcan icon.
  2. To bulk delete medications use the check box then select the trashcan icon.

13. To began documenting Vital Signs select the +Add button.





14. From the modal enter any pertinent information using the fields.

15. Once vital signs have been documented you are able to edit or remove.
  1. From the Actions column you can edit by selecting the pencil icon, to delete select the trashcan icon.
  2. To bulk delete vital signs use the check box then select the trashcan icon.

16. To obtain patient signature select the icon within the Actions column.

17. From the signature modal document the reasons for signing using the dropdown/list view, enter the date and time then have them sign within the signature field.
  1. Select Save after obtaining signature.





18. If patient is not able to sign select the Signature Not Obtained button.

19. From the dropdown/list view select a reason why the signature could not be obtained then select Save.





20. To add any addition signatures select on the +Add button.





21. Complete the information on who is signing.

22. To remove any signatures use the check box and then select the trashcan icon.

23. When the Cardiac Arrest toggle is enabled two additional fields populate.
  1. Use the dropdown or list view to document:
    1. Cardiac Arrest
    2. Initial Arrest Rhythm

24. Continue documenting any additional information.

25. Using the Narrative field to type the narrative or if your agency has templates created select on the Use Template button.

26. To view templates that your agency has created select the expand arrow.

27. Using the check box to select which template you wish to use then select Apply.

28. Once all information has been entered select Save.
